Practice Procedures

1) Communication
Clients and prospective clients are welcome to contact me by phone, 317. 767. 0273, leaving a message via my confidential voice mail.  My voice mail is 24/7 and checked regularly.  If you request a brief phone contact between scheduled appointments, I will respond during my regular business hours currently Tuesday through Saturday. Exceptions to phone availability are announced in advance – i.e. vacation, emergency travel, etc.

2) Client Privacy
Client authorization is required via a signed Release of Information document in order for me to make contact with any client medical or psychiatric staff and/or family members or friends. Contact made by a non-authorized family member or friend to me is an option, with the caveat to be noted that said contact will be communicated to client during next scheduled session.

3) Personal Safety
Any client threats of harm to self or others made to a family members or friends should be immediately reported by calling 911 or the Indianapolis Crisis Line at (317. 251. 7575), with intentions to support client access of safety – i.e. hospital Emergency Room. The client’s prescriber should also be contacted to provide direction re: inpatient treatment, commitments and/or outpatient treatment services. I may also be notified to provide follow-up outpatient treatment. Again, any communication by me with client care team requires a signed Release of Information document.
