A. Insurance coverage is available with Anthem Blue Cross/Blue Shield, United Health Care/Optum, New Avenues, and some Employee Assistance Programs (EAP). Some insurance coverage may also be provided even when Out-of-Network. Insurance coverage is subject to Deductible and Co-payments, which may vary.
B. Any out-of-pocket or self-pay fees are available on a sliding scale, and rates are determined by the client based on financial and scheduling considerations.
C. Payment of fees available in cash or check form only.
